Need to apply for a vendor number, reactivate your vendor number, or change your direct deposit information? We recently updated our vendor number application, vendor number reapplication, and direct deposit change forms.
You’ll find the new lawyer vendor number application and vendor number reapplication forms on our Apply to be a legal aid lawyer web page. If don’t have a vendor number and want to start taking legal aid cases, see Form-A. If your vendor number has been deactivated, you’ll need to apply to reactivate your number and use Form-B. You can also find the reapplication form, along with more details, on our Return to legal aid work webpage.
You can also find these forms, including Form C to update your direct deposit information, on our Lawyer forms webpage and on our Lawyer resources webpage under the Forms section.
Please note: You do not need to fill out these forms if you’re already an active vendor and your direct deposit information has not changed.
