The new two-page Extended CFCSA Services Opinion Checklist is a fillable PDF and replaces the former Extended CFCSA Services Opinion Checklist. We encourage counsel to use this new, shorter template, available through the Lawyer Portal under Resources, Forms, and CFCSA.
